Qualifications of the PGY1 Resident

  • Resident must be a graduate of an ACPE accredited Doctor of Pharmacy degree program
  • Residents must be licensed or be eligible for licensure in IL
  • Residents shall participate in and obey the rules of the Residency Matching Program

Application to the Swedish Hospital Residency Program

Submit the required information to PHORCAS by the deadline:
  • A Letter of Intent including a statement of professional goals and reasons for pursuing a PGY1 Pharmacy Residency
  • Curriculum Vitae
  • Three Letter of Recommendation Forms
  • Official transcript from an accredited School/College of Pharmacy
