Diabetes Self-Management Education and Support Program
Our Nutrition and Diabetes Center (NDC) offers comprehensive education, resources and support to patients diagnosed with type 1 or type 2 diabetes, prediabetes and gestational diabetes.

Our diabetes self-management program is accredited by the
American Diabetes Association
and provides patients with high-quality education to manage their diabetes. Whether you are newly diagnosed with diabetes, are struggling to control the condition or would like an update on the latest research, medications and trends about your condition, we are here to assist you.
Diabetes services offered at NDC include:
One-on-one or group diabetes education sessions to help you learn more about the types of diabetes, diabetes symptoms and diabetes treatment
Diabetes diet and nutrition counseling
Insulin pump management education
Continuous Glucose Monitoring (CGM) education
Diabetes support group

More About Our Program
Our Diabetes Care and Education Specialists work with your primary care physician to help you achieve optimal control of your diabetes. By providing comprehensive education and information about self-care, we can help you decrease complications and improve quality of life.
Our diabetes education sessions cover a wide range of topics, including but not limited to:
Types of diabetes
Disease process and progression
Healthy eating to manage diabetes and what foods should be avoided with biagetes
The importance of physical activity
Medication use and safety (oral, insulin and insulin delivery systems)
Blood glucose monitoring including use of glucometers and CGM.
How to prevent, detect and treat acute and chronic complications
Goal setting and problem solving
Psychosocial adjustment
Preconception care, management during pregnancy and gestational diabetes
Physician Referrals and Insurance Coverage
A physician’s referral is needed for diabetes education services.
Commercial Insurances and Medicare typically cover the cost of diabetes education. We offer free diabetes education sessions for patients with diabetes or prediabetes who are on Medicaid or uninsured.

Why Receive Nutrition and Diabetes Education at the Nutrition and Diabetes Center?
Decreased A1C levels
In FY 2021 69% of patients showed a drop in their A1C level after attending education sessions.
Improved eating habits
In FY 2021 80% of patients showed an improvement in their eating habits after attending education sessions.
Continued Support
At least 50% of the patients attended a follow-up session after their initial appointment
